How to get a GLP-1 without insurance in the US

Updated: 2026-10-17

According to KFF, 1 in 8 US adults say they currently take a GLP-1 and half say it is hard to afford. If you have no insurance (or it won’t cover it), these are the routes. All require a prescription. This is not medical advice: which drug and dose is right for you is a licensed clinician’s decision.

1. Manufacturer self-pay programs (brand, FDA-approved)

2. Telehealth membership + brand medication

Some platforms charge a monthly membership plus medication. Example: GoodRx charges a $39/month membership and sells Wegovy at offer prices (source). Always add membership + medication.

3. Compounded medication programs (cash pay)

Many clinics sell compounded semaglutide or tirzepatide. They cost less but are not FDA-approved and are not the same as brand-name drugs (see Brand vs compounded). In October 2026 a public index found a median of $189/month for compounded semaglutide and $249 for compounded tirzepatide (MatchScript).

4. Public coverage

Some state Medicaid programs cover GLP-1s for obesity with requirements, and a Medicare GLP-1 bridge program runs through Dec 31, 2027 (per GLP-1 Watchdog). Ask your Medicaid or Medicare office.

What is the cheapest option?

It depends on your state, the medication and the dose your clinician decides. Use the comparator to see the published all-in monthly price and confirm with the clinic.
